The Community Wellness Program is a holistic, community-centered initiative designed to promote health, safety, and resilience across neighborhoods through integrated services and proactive engagement. Our model bridges healthcare, outreach, and civic involvement to address the physical, mental, and social well-being of individuals and families.

Our Events

The Community Wellness Program engages the community through a variety of impactful events:

Community Meetings

Open forums for residents to share concerns, access resources, and stay informed about local initiatives take place every third Tuesday of the month.

Senior Engagement Events

We host Senior Game Day every second Friday and Steppin’ With The Seniors every last Friday of the month to encourage social connection and wellness. In addition, our new Senior Chair Exerciseinitiative offers convenient, low-impact fitness sessions on the first Thursday of each month.

Prayer Meetings

Held every Wednesday, these gatherings offer spiritual encouragement, community connection, and a space for collective healing and reflection.

Mobilizations

Rapid response and neighborhood activation around urgent community needs such as violence prevention or health outreach.

Block Club Meetings

Grassroots collaboration that strengthens neighborhood safety, unity, and communication.

Home Wellness Visits & Street Outreach

Personalized, door-to-door engagement to support individuals and families with direct services and resources.
